Perimenopause, Weight and Waistline: Why Can Body Shape Change After 40?

‘My weight has not changed very much, so why do my trousers feel tighter around the waist?’ After 40, this may reflect an overlap of ageing, hormonal changes, muscle, activity, sleep, food patterns and genetics—not one cause alone.
1. Why might abdominal fat increase?
Malaysia’s menopause guideline notes that visceral fat around the abdomen may begin increasing several years before menopause. Hormonal changes can influence where fat is stored, while overall weight is also shaped by ageing, activity, eating patterns, sleep and genetics.
2. Why does the scale not tell the whole story?
Weight does not show how much muscle or fat you have, or where fat is stored. A woman can remain at a similar weight while losing muscle and carrying more abdominal fat. A waist measurement does not diagnose disease, but excess waist fat is associated with metabolic risks.
3. How can you track change without being controlled by numbers?
Notice longer-term weight trends, waist measurements taken consistently, strength in everyday movements, sleep, stress, activity, menstrual changes and health numbers when tested. The aim is to recognise a pattern, not judge your body.
4. Foundations worth protecting
Build strength and daily movement gradually, choose an eating pattern you can sustain, and include sleep and stress in the picture. Seek medical advice for rapid unexplained weight gain or concerning symptoms such as swelling, breathlessness or persistent abdominal enlargement.
A changing body after 40 does not deserve shame. Strength, sleep, health measurements and daily wellbeing matter alongside weight.
